Prior to proceeding with the Faith Formation registration form, anyone who was NOT baptized at Our Lady of the Lake church, must send a copy of your child's Baptismal Certificate (unless you have already done so in a prior year) to: [email protected]
or mail to: Our Lady of the Lake Parish, 294 Sparta Ave., Sparta NJ 07871.

If you cannot find your child(ren)'s Baptismal certificate, please reach out to the parish they were baptized at for another copy before registering.
